Men for Plastic Surgery Becoming More Common
With cosmetic surgery no longer taboo and a workplace focus on looking young, more men go under the knife.

When John Tlapa looked in the mirror, his nose looked like he “could pick a door lock,” he said. It resembled “a hook with a point on it. It was pretty ugly." So two years ago, the San Diego-based screenwriter underwent rhinoplasty to improve his profile and fix a deviated septum that had plagued him for almost 40 years.
Tlapa, 54, is part of a trend that, in recent years, has seen increasing numbers of men seeking cosmetic surgery. In 2011, 9% of surgical and nonsurgical cosmetic procedures in the U.S. were conducted on men, according to the American Society for Aesthetic Plastic Surgery — a 121% increase since 1997.

While the number of cosmetic surgical procedures performed in the U.S. increased just 1% from 2010 to 2011, the number of males getting liposuction and eyelid surgeries is on the rise. More than 41,000 lipoplasty procedures and almost 23,000 eyelid surgeries were performed on men in 2011, increasing 14% and 6%, respectively.
Women in recent years have been trending toward minimally invasive cosmetic treatments such as Botox and Restalyne, but the fastest-growing cosmetic procedures for men are surgical. Liposuction tops the list of most common cosmetic surgical procedures performed on men, followed by rhinoplasty, eyelid surgery, male breast reduction and face-lifts.
“The taboo is lifting for men,” said Renato Saltz, a board-certified plastic surgeon in Park City, Utah, who is also vice president of the International Society of Aesthetic Plastic Surgery. “In the past few years, plastic surgery is everywhere. All those nip-and-tuck shows have brought plastic surgery to a common ground.”
As recently as a decade ago, “Patients would come in, have their surgery and hide for eight weeks. Nobody would talk about it,” Saltz said. “Now you have your surgery in the morning, and in the afternoon you’re talking to your friends. It’s become a much more acceptable commodity.”
A combination of medical, technological, economic and societal factors are contributing to the increase. Advances in anesthesia, in particular, have made many procedures available on an outpatient basis, negating the need for overnight hospital stays, reducing recovery times and allowing patients to get back to work more quickly. Just 18% of cosmetic surgeries were performed in hospitals last year, according to the ASAPS. The prevalence of makeover shows on television, and the wealth of information available over the Internet, has also contributed to plastic surgery’s acceptance.
But the largest driver of the trend is the simple fact that such a sizable portion of the U.S. population is aging. People ages 35 to 64 account for 71% of all cosmetic procedures in the U.S., the country with the largest number of cosmetic procedures performed, according to the 2011 International Study of Aesthetic/Cosmetic Surgery Procedures.
“There’s an increased trend of men over the last three or four years because of the economy,” said Dr. Babak Azizzadeh of Beverly Hills, a Harvard-trained plastic surgeon who specializes in cosmetic and reconstructive surgery of the face, eyes and nose. “It’s requiring that people look younger and more youthful. In the past, CEOs had to have a lot of gray hair. Now you see Mark Zuckerberg, these big entrepreneurs who are young. The reason eyelid and abdominal liposuction surgeries are so popular is because the eyelids and midsection are among the first parts of the body to show signs of age and among the most difficult to remedy without surgery,” Azizzadeh said.

New Study Shows Lipoabdominoplasty is Safe, Effective

Lipoabdominoplasty can provides excellent patient outcomes with a low complication rate, according to a study in Plastic and Reconstructive Surgery-Global Open.®
In
the new study, 150 patients who underwent Iipoabdominoplasty were
compared to 384 patients who had liposuction alone and 17 patients who
had abdominoplasty. The combined technique was designed to reduce
surgical trauma. Patients initially underwent ultrasonic liposuction in
the abdomen and flanks. Liposuction was followed by abdominoplasty.
"Liposuction and abdominoplasty, individually and in combination, may be performed safely with appropriate measures to reduce complications by minimizing tissue trauma," concludes study author Eric Swanson, MD, a plastic surgeon in private practice in Leawood, Kansas.
Notably, no liposuction patients developed seromas. There was a 5% rate of seromas after abdominoplasty. Swanson writes that seromas have led some plastic surgeons to leave behind some abdominal fat and connective tissue during abdominoplasty. He believes that preserving this excess tissue is unnecessary and compromises the cosmetic result.

Basic Safety Measures Reduce Risk of Combined Procedures
The study demonstrates that by using shorter periods of ultrasound and eliminating the traditional use of electrodissection during surgery, the risk of seromas may be minimized without sacrificing the cosmetic results. By first injecting the tissues with fluid containing epinephrine, blood loss is reduced, making cautery dissection unnecessary. In the report, Swanson also describes a modified form of general anesthesia—total intravenous anesthesia without muscle paralysis—to reduce the risk of blood clots in the legs. Only one patient in his series developed this complication and was successfully treated. He writes that by adopting safe preventive measures, surgeons may reduce the risk of this serious complication without the need for blood-thinning medication. Patients also recover more quickly after surgery, averaging only 51 minutes in the recovery room, and experience less nausea.See the original article.

The Unexpected Benefits of Botox

Botox, Xeomin and Dysport are all used in countering wrinkles. They are a purified form botulinum toxin A, which means there is not a botulism risk when the treatment is used properly. They work by blocking the nerves that contracts the muscles which soften the appearance of wrinkles.
The Benefits and Side Effects
When using Botox, it is most effective on wrinkles that haven’t completely set in -- “dynamic” wrinkles that appear when you move your face, such as frowning. “If you don’t move the muscle too much, you will not form the wrinkle,” states Columbia University dermatologist Monica Halem, MD. Dr. Halem considers Botox as preventive.
The FDA reminds that you talk to your doctor before starting Botox, Dysport of Xeomin if you are breastfeeding, pregnant or plan to become pregnant.
Some side-affects are possible: headaches, pain at the injection site and minor bruising. Less than 1% of cases, drooping eyebrows or eyelids occurred and returned to their natural state within a few months.

If you decide to try Botox:
- Avoid medications that can thin the blood. If you are taking aspirin or ibuprofen, you are more likely to bruise at the site of the needle injection. It is recommended that you avoid taking these types of medication for two weeks in advance to your Botox treatment. Ensure you inform your doctor before treatment about any supplements you are taking, even if they are “organic.” Some of these supplements (such as ginko, vitamin E or fish oil) also thin your blood. Your doctor may consult you to not use these supplements for two weeks prior to your Botox treatment.
- Take it easy. Ask your doctor or nurse for a more natural, expressive look. Your doctor can adjust your doses accordingly.
- Watch the partying. Parties and spas are fun, but not for injection fillers. Always be sure your Botox is performed by a doctor who is board-certified in plastic surgery or dermatology.

Today's question is:
I have had a breast augmentation. I am currently breast feeding and my
natural breast tissue due to enlargement with lactation is giving a double bubble
effect over the implant. Have you seen something like this before?
The increased volume from engorged breasts can either create
the illusion of a double bubble or cause the implant to drop below the IMF if
the fold has been violated. You just have to sit tight and wait for you
lactation to stop and your glands to regress. Then we can know for sure.
